Registered Nurse - Supervisory Care Manager
Duties include but not limited to:
The Supervisory Registered Nurse (RN) Care-in-the-Community Clinical Manager will be assigned to Community Care Service, which is aligned under the office of the Chief of Staff, under general supervision of the Chief of Community Care. The Supervisory RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of Community Care nurses. The Supervisory RN will manage episodes of care that have been referred into the community (either through the MISSION Act, or through traditional Non-VA Care). The Supervisory Registered Nurse will serve as supervisor for all Community Care nursing staff and act as a liaison to both internal and external providers by sharing clinical information in accordance with VA privacy rules. He/ She will be responsible for reviewing records and ensuring seamless transition to and from the community. Incumbent possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy. Core elements of performance include first-line supervisory authority, responsibility, and accountability for the clinical operations of Community Care service RNs and LPNs, ensuring that subordinate personnel provide timely coordination of care which complies with generally accepted standards of clinical practice. The nurse's primary commitment is to the patient, whether an individual, family, group, or community. The Supervisory RN will supervise, assess, and coordinate clinical patient care needs for Community Care Service. Work Schedule:

The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required up to 50 pounds. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. In addition, the incumbent must be emotionally stable and adjust to different types of personalities, demonstrating cheerfulness, compassion, courtesy and concern. He/she must be quick and dependable in taking emergency action as directed and use initiative in preventing accidents.
