Registered Nurse-Case Manager

Responsible for providing timely coordination of care which complies with generally accepted standards of clinical practice. Assess, develop, implement, and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with internal and external sources. Assess the appropriateness of the level of care, diagnostic testing and clinical procedures, quality and clinical risk issues, and documentation of Community Care medical record completeness. Reviews, analyzes and makes preliminary decisions on all treatment plans, or requests from private physicians. Approves treatment plans and requests additional information as required according to approved Standardized Episodes of Care. Provides direction and coordination of all aspects of the movement of Veterans between one or more VAMCs and community healthcare facilities, to assure movements occur in a manner that is conducive to the efficient and effective operation of clinical continuity of care, as directed and/or assigned; i.e. Inpatient, Outpatient, IFC/IFT, Alexandria Consult Management. Enters requests for care received from community providers into the consultation package of CPRS and makes recommendations for continuation or referral to government resources on it. Reviews physician's progress notes and orders to determine that adequate and timely documentation exists to indicate the medical necessity and appropriateness of outpatient care. Reviews patient data for utilization issues and confers with utilization expertise regarding specific cases. Requests from health care provider's documentation of services provided. Consults appropriate physician specialist for all cases where treatment plan is not approved as written. Communicates continually with patients, families, medical staff, caregivers, and third-party payers as necessary. Demonstrates awareness of legal issues in all aspects of patient care. Develops and maintains a positive work climate and supports the overall team effort Demonstrates an awareness of self, responsibility, and accountability of own professional practice. Practices capable and effective problem identification and resolution skills as a method of sound decision making. Demonstrates highly effective communication methods and skills, using lines of authority appropriately. Performs other duties as assigned or directed. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
Minimum of 1 year of experience as a Registered Nurse, previous case management experience is preferred but not required. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially prolonged periods of 8-12 hours of continued walking, standing, stooping, sitting, bending, pulling, and pushing. This position requires periods of light, moderate, and heavy lifting from less than 15 pounds to 45 pounds and over. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Reference:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
