Position Hours:
Part-time Position Qualifications & Requirements:
Valid Louisiana LMSW or LCSW license required 1 year of experience Ability to meet physical demands of the job Positive attitude Dedication to quality patient care Strong communication and critical thinking skills Willingness to develop working knowledge of Federal and State Regulations/Conditions of Participation as they relate to the referral and admission process for Hospice.
Ability to work well with families, physicians and facilities.
Basic computer skills.
Punctual, reliable and available during routine scheduled hours.
Occasionally duties may include hours outside of these.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Willingness to participate in appropriate education as requested by immediate supervisor.
Contribute to team effort by accomplishing related results as needed Prepare clinical notes Other duties as assigned
Responsibilities:
Oversee and provide patient care visits.
Develop social worker patient plan of care.
Coordinate with patient's family, physician and caregivers.
Completes and submits timely clinical documentation that is accurate, comprehensive, and reflective of the appropriate terminal condition.
Communicates with other team members and participates in the IDT process.
Develops the initial and ongoing plan of care with collaboration from other team members.
Promotes Guardian Hospice Care in the community.
Supports the Guardian Hospice Care Mission and Values.
Completes other projects and duties as assigned.
